Official Records and Transcript Requests

Requests for Official Records (including transcripts) must be made in writing utilizing the DOE Student Records Request Form

For your convenience, the school information for Long Island City High School has already been filled out in the above forms.

Please note that after the age of 18, only students / former students are allowed to request and pick up their records. The fee for transcript requests is $3.00, and payment can be made in cash or money order made out to Long Island City High School.

Requests for Education Verification

Requests for education verification must be made in writing, and must include the following information:

  • Name

  • Date of Birth

  • Copy of the Signed Authorization Release of Records

  • Year of Graduation / Discharge from Long Island City High School

  • Self-Addressed Stamped Envelope

Requesting a School Letter

To get a face-to-face letter from NYC Public Schools, log in to your NYC Schools Account (NYCSA) to request it electronically, or print and fill out the Student Records Request Form and submit it in person at the school with your photo ID. A parent or guardian must pick up the letter in person and show a valid photo ID. Please feel free to also request these letters in person at the school or by phone. Students under 18 are not allowed to pick up the letters.

Method 1: Using your NYC Schools Account (NYCSA)

Create or log in to your account: Visit https://www.schoolsaccount.nyc/ to create an account or log in if you already have one. You'll need a code and your child's 9-digit student ID number.

When you log in, you can access and request the face-to-face letter through the NYCSA platform.

Please pick up the letter: You still need to pick up the letter in person at the school.

Method 2: In-Person Request at the School

Please fill out the form: Print the Student Records Request Form and complete it with your child's details. Gather required documents: Prepare your photo ID and the completed request form.

If you do not have a completed Student Records Request Form, you will need to complete one in person at the school.

Visit the school: Go to the school in person with your completed form and photo ID.

Pick up the letter: A parent or guardian must show a valid ID matching the school's records to receive the letter.
